Parent/guardian must notify the school nurse of student’s specific allergy.
• Parent/guardian complete Food Allergy Information and Consent Form.
• A Food Allergy Action Plan (Emergency Care Plan) must be completed by the parent/guardian and physician.
• If medication is prescribed a Parent Request for Administration of Medication form must be completed by parent/guardian.
• For food allergies and/or if food substitutes are needed, the Food Allergy/Disability Substitution Request form must be completed by the physician and parent/guardian and returned to the cafeteria manager and a copy must be given to the school nurse.
• Special accommodations may be provided upon recommendation of a core team consisting of parent/guardian, teacher, school nurse and school administrator after receiving documentation from a physician regarding the allergy.
• If special accommodations are needed as recommended by the core team, a room may be designated for students with a Peanut/Nut Allergy and Peanut/Nut Free Awareness Zone signage will be posted at the doorway upon entering the classroom. Students assigned to the Peanut/Nut Allergy classroom must not bring any peanut/nut products into the designated classroom.
• If needed, FISD will provide a designated table in the cafeteria with signage on the table that states it is a Peanut/Nut Free Awareness Zone. A student must have been diagnosed by a physician and the school nurse must have received documentation in order for the student to sit at the designated table. Please see below for* COVID 19 changes .
This school year students may be eating in the classroom or cafeteria, and parents can choose if they want their child to eat at the peanut-free table if eating in the cafeteria. If students eat in the classroom and parents do not request a peanut-free classroom and the parents do not want their student to eat with other students, the teacher can choose an area of the room for the student to sit.

• All FISD students, staff members, and others that support our educational community share the responsibility for preventing bullying and social isolation of children with food allergies. All are encouraged to report bullying and harassment of any child with food allergies to the campus administrator.
• An Anaphylaxis Individual Healthcare Plan (IHP) will be required for students requiring special accommodations or medication.
• Anaphylaxis sheet will be share with school personnel who have a need to know.
• Life-Threatening Allergies Awareness Training is done yearly for all staff .
Food Allergy Action Plan - This plan lists the specific food allergies which may cause life-threatening anaphylaxis, and notates required treatment of Epinephrine administration. Required to be signed by parent and physician. Please bring in individual child's prescribed Epi-Pen (and Benadryl if indicated) to campus as soon as possible.
Food Allergy Consent/Waiver - Required for any documented peanut allergy. You may consent or decline. Does not require doctor's signature.
Food Allergy/Disability Substitution Form - The Food Allergy/Disability Substitution Request form must be completed in its entirety and signed/dated by the student’s physician. This form needs to be returned to: Frisco ISD Child Nutrition, Attn: District Dietitians, 12050 Rolater Road, Frisco, Texas 75035, or by email to: specialdiets@friscoisd.org You do not need to provide a new form each year unless there is a change in your student’s allergies. It is your responsibility to renew this form should your child’s nutritional needs change. To remove allergy restrictions from this student’s account, you must submit a signed note stating the student no longer has the food allergy or intolerance. The "Action Plan" must be completed and signed by you and your child's doctor. Please be specific about the medication and amount of medication your child needs. • Have a “no food sharing” or “no food trading” rule.
• Encourage hand washing after food handling and eating. Liquid soap, bar soap, and sanitizing wipes effectively clean hands of potential allergens, but antibacterial sanitizing gels do not.
• Wash surfaces after food is eaten or used. Commercial wipes, spray cleaners & the use of microfiber towels are most effective at removing peanut protein from tables and other surfaces.
• Use nonfood items for classroom projects, academic rewards and classroom celebrations.
• Encourage parents to send fruit & raw vegetables as snacks.
• Keep “safe snacks” in the classroom for unplanned events.
• Have students store their lunches in a specific location, outside of the classroom.
• Make sure your subs are aware of the child’s Food Allergy and Anaphylaxis Emergency Care Plan. Leave a list of medical alert students in your sub binders. Tell them about the medical alert icon in eschool.
• Training is provided all staff regarding the proper use of Epinephrine and the signs of anaphylactic shock.
