Note that some of the forms will require a doctor's signature as well as a parent's signature. New forms must be completed at the beginning of every school year. They do not carry over from year to year.

PLEASE DO NOT LEAVE YOUR CHILD AT SCHOOL WITHOUT THEIR LIFE SAVING MEDICATION.

ALL medication must be kept in clinic. Please do not send your child to school with medicine to keep in their pocket or backpack--it is not allowed. Medication that can't be given at home can be given during school hours in the clinic at a specific time. If you would like to keep any RX medication at school for your child, please print and complete the "Medication Administration Request Form" below and bring it to the front office along with the medication in a gallon zip lock bag please. Put your child's name, school ID# and grade on the bag. I will meet you in the foyer if you need to speak with me .
Please note that this year only Tylenol, Motrin , plain Benadryl ( not combined with other ingredients) and Tums are the only District approved medications for school and don't require a physicians order / note to give UNLESS your child will need more than 10 doses then I will need a note from their doctor indicating the need for multiple doses while at school or at 5th grade camp . If your child will hit 10 doses on campus before camp , get the note ! All medications must be in their original container, the smallest volume size , children's strength and not expired. All prescription medications must have the prescription label attached. Please see "FISD Medication Policy" page of this website for more information on what medications are allowed at school. Feel free to contact me any time you have questions regarding this policy. I know it's confusing. I'm here to help! I would much rather you ask in advance than bring medicine to school and find out that you'll need a doctor's note before we can use the medication at school. However, if is it not on the approved list, the rule of thumb is to provide a doctor's note or arrange a time to administer the medication at home outside of school hours instead.
FISD policy prohibits me from administering any medication unless I have a completed {by you, and if necessary your child's doctor} form on file for the current school year for the medication. Every year there needs to be a new form signed. A handwritten note from home, email, text, or phone call is not allowed or accepted.
Please note that I cannot keep any medication in the clinic over the summer. All medication must be picked up by parents no later than the last day of school. FISD does not allow me to send medication home with the students. All medication not picked up by that date will be disposed of in accordance with FISD policy without exception.
Please print and complete all of these forms if your child has Asthma. The Action Plan must be completed by your child's doctor. You and the Doctor must both sign this form. Please be specific about the medication and amount of medication your child needs {For example: "Two puffs of Albuterol inhaler every 4 hours as needed for Asthma symptoms"}. All prescription medications must be in their original container with the prescription label attached. Check to be sure the inhaler is not expired prior to bring it to school. THE INHALER NEEDS TO BE INSIDE IT'S BOX.
The "Self Administration for Asthma or Anaphylaxis Medication" form is only needed if your child's doctor feels that your child has the knowledge/maturity to carry and self-administer their own inhaler, usually evaluated in middle school . This will not apply to any student this year . All inhalers will be kept in the clinic and given to the student by the nurse. The form is not necessary to be signed by you, and your child's doctor. All other inhalers also will be kept in the clinic. I will only be able to assess your child if they use their inhaler in the clinic. If they use it on their own , they have not been seen by the nurse.
*Discontinued use of Nebulizer treatments in the school setting
● The AAP (American Academy of Pediatrics) recommends Asthma treatments using inhalers with spacers are preferred over nebulizer treatments whenever possible during COVID-19. This will include school .
"Avoid use of nebulizers as they spread droplets and are not advised. Nebulizers are often not required for children over the age of 6 (MDI inhalers are equally effective in delivering medication). Viral droplets persist in the air for 1 – 2 hours. Administer medication at school using individual inhaler and spacer/valved holding chamber". Source: Allergy and Asthma Network
● Nebulizer treatments in the school setting will no longer be provided. This can also be connected to a face mask and held in place by an adult/care provider/the child, the child can then sit and take several breaths over a period time if unable to use an inhaler in the traditional sense.}
● Parents should send an inhaler chamber/spacer to the clinic for use in the school setting. If your child does not have an inhaler, we encourage you to contact your doctor to ask them to prescribe one for the school setting.
In addition, we will NO longer provide emergency Albuterol via Nebulizer treatments in our clinics as part of our standing orders. Your child may at the evaluation of the nurse / staff have EMS respond to school . You will be notified.

Emergency plan should be completed in detail. Rescue medication must follow prescription rules (be sure dose matches what is written in your plan, not expired and script label matches the plan). Please bring the medication and action plan to the front office in a zip lock bag with the students name , school ID# and grade written on it.
