Please fill out the following application if you believe you are eligible for an honor cord in any department. Use one application for each department in which you are applying for an honor cord. Specify the department on the line provided. You must attach an unofficial high school transcript. These can be obtained from Mrs. Hughes. Applications will be reviewed by each department for qualification. Return this form to the FHS Counseling Center by Friday, April, 2025. These applications will go to the respective department chairs the week of April 7, 2025. Work in the Black Knight Branch of First State Bank
BKTV Studios: (RED & GOLD)
Red: Completed 3-4 years and/or 2 years in a leadership position
Red and Gold: Completed 6-8 years and/or 3 years in a leadership position (Journalism and TV combined)
No grade lower than a B
Free of discipline problems in BKTV Studios Classes
BIO-MEDICAL: (BURGUNDY AND NAVY CORD)
Must complete 4 years in the Biomedical Program at Farmington High School.
Minimum combined grade of A (90% or higher)
NOTE: The only exception is PBS (the substituted Biology course for Biomed students).Any class used to meet another honor cord requirement may not be used for Bio-medical and vice versa.
BLOOD DRIVE: (RED CORD)
Must participate in 3 blood drives during senior year. See Mrs. Roth.
BUSINESS: (DRAB CORD)
Earned minimum of 6 semesters of business classes earning a grade of at least A-
Cumulative GPA of 3.0 or above
Must have been a member of FBLA at least one year and participated in competitive events at least once

CDA PROGRAM: (PURPLE AND WHITE)
Completion of two year program through Mineral Area College
COLLEGE: MAC ASSOCIATES DEGREE HONOR CORD: (CRIMSON)
Automatic! We will receive a list from MAC of students who qualify.
COMPUTER SCIENCE PATHWAY: (SILVER CORD)
Successfully completed 2 PLTW Computer Science Classes with a grade of “A”.
CNA: (RED AND WHITE)
Completion of the Farmington High School Certified Nursing Assistant Program
Be recommended by CNA Instructor
Must successfully pass state certification exam
DRAMA: (BLACK & WHITE CORD)
Must participate in 5 FHS Theatre productions, or provide documentation of other productions outside of FHS.
Complete 4 semesters of the following courses (Drama, Adv. Drama, College Public Speaking, Cinema and Literature, Stage Makeup, and summer production)
Be a member of good standing with FHS Theatre Guild
