Enrollment Procedures for Farmington High School

Please click on "register new family" and follow the prompts.
Before any student can be enrolled we must fax a request to the student's former school. They will send us an "official" transcript which includes: all high school classes taken with grades, discipline records, attendance records, immunization records, assessment scores/MAP/EOC testing, MO and US Constitution results, transfer grades, A+ information (if applicable), special services information (if applicable).
Once we have received the official transcript, the school registrar will contact you to schedule a time and date for you to meet with the student's counselor and enroll. Both the student and the parent/guardian must be present at this appointment.
You must bring the following information with you to this appointment:
Proof of residency (something that validates your physical address). The document must show both your name and address. Such as, utility bill, property tax receipt or rental agreement. Drivers license or general mail will not be accepted.
If the student does not reside with the parent then you MUST provide us with legal documentation showing you have legal guardianship of the student. Power of Attorney is accepted.
THIS DOCUMENTATION WILL REQUIRE ADMINISTRATIVE APPROVAL
