Teachers will receive a tentative class list of students found eligible through screenings or referral/IEP process for the upcoming school year prior to the end of the current school year when possible. Often, our families are difficult to contact. Three different attempts to enroll should be made prior to moving on to the next student (phone call, text, email, letter, drive by, etc.). If and when spots open up within a Project HELP classroom, students will be added from the wait list maintained in the Early Childhood Office based on the highest number of points and the greatest need or as determined by the IEP team.
Each classroom should hold open a minimum of 2 spots in each session until October 1. This will allow the screening staff to conduct screenings and ensure there will be openings for students who have been found eligible for and IEP. Wait list procedures will be followed, providing for students with special needs and those found to be the most at-risk until all spots are filled. Recruitment will occur year round in order to maintain total enrollment.
Children who are eligible for kindergarten (five or older on September 1 of the program year) cannot be enrolled in the program. Children will not be excluded from participating in Project HELP if they are not toilet trained. Every child who is enrolled within a Project HELP classroom must have a file with the following documentation included:
Name, address, phone number
Birth certificates indicating age eligibility (certified - not the one from the hospital with child's feet prints) DO NOT KEEP ORIGINAL - MAKE A COPY FOR FILE
Physical and immunization records (GIVE A COPY OF THIS AND BIRTH CERTIFICATE TO SCHOOL NURSE). This MUST BE SIGNED by provider and parent. File is not complete unless both signatures are present.
Vision and hearing screening results
Demographic and family information, including emergency contact information and names with contact information of anyone to whom the child can be released to
Proof of family income
A behavior support plan, if applicable
A program transition plan, if applicable
If a parent/guardian is experiencing difficulty obtaining any of the required items, please let Laura Dunn, Parent Coordinator, know and she will work with the families to get what they need.
Income verification is a requirement to participate in Project HELP. Below is the income verification form as well as guidance put out by ISBE with the Income Verification FAQ document. Income levels associated with each public benefit are below to assist with documentation. AT NO TIME SHOULD ANY OF THE DOCUMENTS COLLECTED TO VERIFY INCOME INCLUDE ANY SOCIAL SECURITY NUMBERS OF THE FAMILIES! The Illinois Identity Protection Act, 5 ILCS 179 protects the confidential nature of the SSN and the risk of identity theft related to to unauthorized disclosure. Please ask families to black out this information prior to submitting should the artifact used for income verification have a SSN on it.
