Project HELP teachers must complete the "Request to Attend Professional Meeting" form any time that they wish to attend a professional development opportunity that is not put on by the Early Childhood Department at EIASE. All items must be filled in on this form prior to submission and any supporting documentation (flyer, agenda, ect.) to be considered must be included with the submission as well. All items should be submitted to the Early Childhood office at least two weeks prior to requesting to attend. Per IRS rules, meals are not allowable unless an overnight stay will occur. The "Request to Attend Professional Meeting" form can be found in the office mailbox at staff.eiase.com.
The convention reimbursement form is used to request reimbursement expenditures that occurred while attending an approved inservice. Reimbursement requests should not exceed the approved amount on the original Request to Attend form. Submit the completed form to the Early Childhood Office for processing. Reimbursement checks will be mailed to staff address on file and will be sent after the expenditure is approved at EIASE's monthly board meetings. Reimbursement claims should be completed within 30 days of the occurrence in order to ensure payment. The "Convention Reimbursement" form can be found in the office mailbox at staff.eiase.com.
Project HELP staff who are employed by the member districts will need to complete and attach the District Staff Inservice Approval form in addition to the Convention Reimbursement Form and submit both forms together to the Early Childhood Office for processing.
