It is the responsibility of the requester to provide or read this information to the child or young person they are requesting assistance for, or to the parent where the child is under 12 years old. 

Children and Young People can refuse for their information to be shared with certain agencies. The requester should make this clear in the body of the referral if that is the case. 

Please note that a child, young person or parent can only provide fully informed consent if they have understood this statement.
