Lastly, before final submission of the Request, you will have the opportunity to review the information you have provided and can go back and amend if required. 

If you are happy with the Request click Submit. 

The Request will be sent automatically to the inboxes of the services requested. You as the requester will also receive a copy and confirmation of the submission. 

Depending on the age of the child and consents provided, the parent and/or child/young person will also receive a copy, including all attached paperwork. 

The services requested will consider your requests individually and will each respond in due course with an outcome.
