Curricular Practical Training Application Form 

Please work through the sections in order and complete all required fields to request Curricular Practical Training (CPT) authorization. The entire CPT application process may take up to 2 weeks as you will need to get the approval of your academic advisor and possibly enroll in a CPT course. We will work with you throughout this process, however, you should note that if any of the requirements are not provided, this may push back your employment start date.

This process has changed slightly due to our Denison ISS Portal software transition. Please follow the instructions carefully. You will be required to email your employer offer letter to Dr. Chad Buckwalter (buckwalterc@denison.edu). He will contact your faculty advisor or academic department for approval of your internship.

Please allow 3-5 business days for processing immigration-related requests. If you need to resubmit this request for any reason, the processing timeline will restart from the date of the new submission.
