English for Academic Purposes (EAP) Program Application Form

Complete this registration form to join the Multilingual Learning Office for the English for Academic Purposes (EAP) program prior to orientation. We'll explore each year you'll be at Denison, from the people you'll meet to the classes you'll take, and work on improving your English along the way. 

**Participants in this program will need to register via this questionnaire by July 1. Participants should plan to arrive on campus on EAP Student Arrival Day as outlined on the New International Students tab in the Information Center. The Center for Global Programs will provide airport transportation from Columbus International Airport to campus for students who have registered for this program.
