Parent Volunteering Form (required)

• • • • Help us get you connected with ways your particular skills can be best used

      • This will get the right volunteer coordinator in touch with you, rather than us grasping at straws

      • Needs to be completed with registration, one per parent if you can

      • You should also complete a CVSD Volunteer Application if you plan to do ANY overnighters, incl. travel/chaperoning

Pre-Participation Physical Form (required)

• • • • Signed by your medical examiner/doctor

      •  You may check the status of your students physical by emailing  Katie Horgan at khorgan@cvsd.org

      • Physicals can be emailed to mhamilton@cvsd.org

      • Cannot expire before November 1st

PE Waiver (optional)

• • • • This must be completed and returned to the office

      • This is not to give you credit for PE, it is to change your required PE Credit to an Elective (Marching Band)

CVSD Volunteer Application

Pride and Honor Activities Code

• • • • For students and families to review, no signature is required

Medical (Authorization for Administration at School)

• • • • Please return to MAIN OFFICE

      • Only if your student plans on taking prescription meds while on campus

Permission for Carrying OTC Medication

      • Only if your student plans on using Over-the-Counter medications on campus
