Indicator 13
What is it?
Indicator 13 is a measurement of the transition plan compliance for students who are transition aged. The transition plan is measured for:
Measurable post secondary outcomes that are updated annually.
Outcomes must be based on age-appropriate assessments, that have been completed less than a year before the meeting.
Transition Services that are provided by the school (i.e. Community Based Instruction, Job Fairs, College Tours, Counseling, etc.).
Course of Study in alignment with the student's post secondary outcomes.
IEP goals that are aligned to meet the student's post secondary outcomes.
What is the process for CPS?
It's important to note that the Indicator 13 process can change yearly, however, recently the process for CPS has been as such:
1. ISBE randomly selects schools and students' transition plans to audit based on data from the Transition page (Prong 1).
2. The Transition Team collects all pertinent documents from selected school teams and submits them to ISBE (IEP/Transition Plan, Consent to Invite an Outside Agency, Notice Of Conference, and any Waivers).
3. ISBE audits the Prong 1 IEPs, using the ISBE Scoring Rubric (see below).
4. Meanwhile, schools that have been selected to be audited, go through mandatory webinar training to build capacity in transition plan writing.
-Once schools have completed the trainings, any IEPs written after the training will be reviewed by a member of the Transition Team. The Transition Team will give feedback and/or approval to finalize. *IEPs cannot be finalized until they have been approved*
5. ISBE returns Prong 1 findings which are scored as pass or fail.
-If they pass: The IEP has completed the Indicator 13 process. No further action needed.
-If they fail: The IEP and all pertinent documents must be revised to reflect compliance.
6. The IEPs that have failed, are resubmitted to ISBE to be audited, after revisions have been made.
7. Once Prong 1 IEPs are revised and pass the audit stage, ISBE selects IEPs for Prong 2.
8. The process is repeated until all IEPs have passed under the Indicator 13 Rubric (See below).

Below is a step-by-step guide to assist you with pulling Profile Page Reports.
Use this as a guideline when completing your high school transition plans. All areas of the checklist should be identified as "completed" in order for the Transition Plan to be deemed compliant by ISBE.
Consent to Invite an Outside Agency
Consent/refusal must be obtained each calendar year.

What is it?
Indicator 14, or State Performance Plan (SPP) 14, requires that all states collect and report data to the U.S. Department of Education. Schools collect data in regards to former students' post secondary education/training and employment outcomes, one year after the student has graduated.
The Process for CPS
It's important to note that the Indicator 14 process can change yearly, however, recently the process for CPS has been as such:
ISBE sends the Transition Team the randomly selected schools and students to pull data.
The Transition Team contacts the identified schools and facilitates completion of Indicator 14 questionnaires in collaboration with the schools.
Schools and their designated point person, then sign up for I-Star Access.
Once schools have I-Star access, the point person(s) begins to call former students to complete a quick questionnaire.
If a student agrees to complete the questionnaire, the point person enters the answers into I-Star.
The Transition Team will additionally mail questionnaires to most recent addresses. A total of three contacts must be made.
Students/Parents do have the option to refuse participation.
If a student is not reached after the third contact, the school has finished the process with that student.
Data is then submitted to ISBE for review.
