Any one entering the building for more than 10 minutes must fill out a health screener
The Health Screener needs to be filled out daily
Parents or guardians must fill out the screener for children under the age of 18 years. Each one of your children attending in-person class will need a screener filled out.
Guardians will receive a text message or email with a link specific to that student's health screener everyday that student is supposed to attend in-person class
You must fill it out the day of in-person learning, you will not be able to fill it out the night before
Please answer all the questions as truthfully as you can
If you get a green screen at the end of your screener, you have to go ahead to attend pending your temperature check at the door
If you get a red screen on your health screener, you may not enter the building and you must remain home
