Registration Process

Welcome to Farnsworth Elementary School! 

We are a neighborhood school and in order to enroll, you must live within our attendance area. Unfortunately, we are not accepting any students who live outside our attendance area. Please click here to see if you live in Farnsworth's attendance area.

If you are interested in registering your child at Farnsworth and you live within our boundaries, please follow the instructions below.

You may stop in at Door 1 on Linder, pick up a printed enrollment packet and drop it off when it is completed.

The enrollment packet includes:

-CPS Enrollment Form

-Request for Emergency and Health Information

-Student Medical Information

-Family Income Information Form

-Race and Ethnicity Survey

-Home Language Survey

-Report Card Language Form

-School Messaging Consent Form

-Media Consent Form

-Student Attendance Policies and Procedures

-Health Records forms

You will also need copies of the following (we can make copies for you when you drop off your packet):

-Copy of your driver's license or state ID; it must have the same address as your proof of address documents.

-Proof of address within Farnsworth’s attendance boundaries (utility bill, mortgage, medical card, etc.).  Please click here to make sure you live in the school's boundaries. We are not enrolling anyone outside our attendance area.

-Copy of your child’s birth certificate
