The Cortland County Community Health Assessment (CHA) and Community Health Improvement Plan (CHIP) were developed by the Cortland County Health Department in collaboration with Guthrie Cortland Medical Center with input from a broad variety of community partners.
Using a survey administered in early 2022 to over 2,000 members of the Cortland community, the CHA presents demographic data and data about health indicators for Cortland County residents. The survey was administered by ## and collected data about health-related factors (such as physical activity, food security, chronic illness, fruit/vegetable intake, drug use, and suicidal ideation).
Based on the data collected via the CHA survey--as well as secondary data and input from local partner agencies--the CHIP team developed the following focus areas and specific goals:
Goal #1: Prevent Opioid and Other Substance Misuse and Death
Goal #2: Prevent Suicide
Goal #3: Prevent Initiation of Tobacco Use and Youth Vaping
Goal #4: Promote Tobacco Use Cessation
Goal #5: Eliminate Secondhand Smoke
Goal #6: Improve COVID-19 and Flu Vaccination Rates
Goal #7: Reduce COVID-19 and Flu Vaccination Coverage Disparities
