NEW APPLICATION PROCEDURE:

1. Access the Google Form and Fill-in the necessary details

2. Upload in the Google form the following documents:

a. Application Letter

b. Student Assistants Bio-data (CNSC-OP-SSD-01F5) [Download below]

c. 2x2 picture

d. Registration form of the current semester

e. Certificate of Grades of the previous semester

f. Certificate of Good Moral Character

3. Submit the hardcopies at the SFAU Office:

a. Application Letter

b. Student Assistants Bio-data (CNSC-OP-SSD-01F5) [Download below]

c. 2x2 picture

d. Registration form of the current semester (Original and 1 photocopy)

e. Certificate of Grades of the previous semester (Original and 1 photocopy)

f. Certificate of Good Moral Character

RENEWAL PROCEDURE:

1. Access the Google Form and Fill-in the necessary details

2. Upload in the Google form the following documents:

a. Renewal Application Letter

b. Student Assistants Bio-data (CNSC-OP-SSD-01F5) [Download below]

c. 2x2 picture

d. Certificate of Good Moral Character

e. Performance Evaluation Criteria (CNSC-0P-SSD-01F7) duly signed by the Immediate Supervisor [Download below]

3. Submit the hardcopies at the SFAU Office:

a. Renewal Application Letter

b. Student Assistants Bio-data (CNSC-OP-SSD-01F5) [Download below]

c. 2x2 picture

d. Certificate of Good Moral Character (photocopy) - bring the original copy

e. Performance Evaluation Criteria (CNSC-0P-SSD-01F7) duly signed by the Immediate Supervisor [Download below]
