AUTHORISED COLLECTION

This form is only to authorise the collection of students by a third party.  Please complete this form and attach the necessary documentation if your child is going to be collected from school or from the bus stop by someone who is not their Mother, Father or legal guardian and who is not named on the enrolment form. 

Este formulario se destina EXCLUSIVAMENTE para autorizar la recogida de los alumnos por otras personas. Les rogamos que rellenen este formulario y que adjunten la documentación requerida si a su hijo le va a recoger en el colegio o en la parada de bus alguien que no sean padre, madre o tutor legal y que así nos consten en la matrícula del colegio.
