All components of the personnel fall protection system will be inspected for defective components daily and documented on Form 350-Fall Protection Equipment Inspection-Daily.
All components of the personnel fall protection system will be inspected annually and documented on Form 350-Fall Protection Equipment Inspection-Annual.
If a harness, lanyard, or lifeline is subjected to a load, it will be removed immediately from service and discarded.
Personal fall arrest systems and components subjected to impact loading shall be immediately removed from service and shall not be used again for employee protection until inspected and determined by a competent person to be undamaged and suitable for reuse.

Complete the Google Form linked in the button titled Register Your PFAS so your kit can be accounted for in the BHI PFAS Inspection Record and given a Kit Number, i.e. BH1 PFAS, BH2 PFAS, etc.

Complete the Google Form linked in the button titled Update Your PFAS information if your kit, or elements of your kit, have been retired and replaced. While the prefilled Google forms can be changed, it is best to have updated information.
