Come and visit the library during office hours (Monday to Friday from 08:00 AM to 05:00 PM) and head to the information desk and seek the assistance of the assigned librarian/library staff and inform them of your purpose: to register/apply for a library card.
Currently enrolled first (1st) year to fourth (4th) year students.

(1) Latest 1x1 formal picture
To know when your library card has already been made and ready for release. Kindly check access here: List of Library Cards for Issuance to see if your name has already been added. Once you see your name in the following list, kindly go to your designated College Library campus to receive your library card.

The following are the required information to fill in via Google Forms:
o Email address
o Name (Surname, First Name, Middle Name)
o Gender: Male or Female
o Address (Barangay, Municipality, Province)
o Student number
o Course
o Student and Guardian’s Contact Number

Fill out all the information details in the Google Forms and 1x1 picture for when you receive your library card for identification.

Once you see your name in the following list, kindly go to your designated College Library campus to receive your library card. All you need is to kindly BRING your latest 1x1 current picture, for your library card.
What to do when you lost your library card?
Currently enrolled students first year to 4th year: Visit the library and request for library card replacement for a fee of ₱50.00.
Recently-graduated students and old students: Visit the library and ask for your assigned library card number to the College Library and head to your local municipal hall or public attorney office to request for an Affidavit of Loss (AOL), once you have your document kindly submit the AOL to the College Library to have your library clearance signed.
