For all your evidence, MLA format insists that you put a label on it that includes a number and a description/caption. Images are called "Figure" and charts are called "Table."
So a photo example might be captioned like this: "Figure 1. Brainstorm worksheet."
An action plan might be captioned like this: "Table 1. Action plan with success criteria."
Also regarding your evidence, you cannot drop it in and expect it to speak for itself. You must refer to it and describe how it supports your claim. You might feel like you're repeating what is in the table, but this is important. Imagine that your reader reads only your narrative and cannot view the evidence, would they still understand how you met the criteria? If not, you need to add more description in your paragraphs.
