Adult Foster Care (AFC) is a program that allows persons above the age of 16 who need assistance with personal care tasks to remain in their home and community with a qualified and approved caregiver. This program works by providing financial assistance and support for family members or friends who act as caregivers. In the AFC program, the caregiver must reside in the same property as the recipient of care.
The benefits of the AFC program should be thought of on two levels:
Benefits to the individual in need of care and the benefits received by the caregiver.
Members receive supervision, and assistance with day-to-day activities, such as feeding, bathing, dressing, and mobility. They can also receive assistance with other activities, such as transportation, and medication reminders. They also benefit by living in a family home with care being provided by someone with whom they are comfortable and familiar.
Caregivers receive tax-free compensation for their caregiving efforts. In the case of family members, caregivers are paid for work that they would have likely provided for free. The AFC Program offers two levels of compensation, depending on the care needs of the individual.
AFC clients are categorized by the level of care they need.
Level 1: For members who require hands-on assistance with 1 or 2 activities of daily living (ADLs)” or require cueing & supervision throughout one or more activities.
Level 2: For members who require hands-on assistance with at least three of the activities or hands-on assistance with at least two activities of daily living and management of behavior that requires frequent caregiver intervention such as: wandering, verbally abusive behavioral symptoms, screaming, cursing, physical abusive symptoms, resisting care

INSTRUMENTAL ACTIVITIES OF DAILY LIVING (IADLs)
Assisting with cognitive or behavioral needs
Maintaining participant safety 24 hours / 7 days a week.
Cueing or reminders to take medications.
Supporting services e.g., doctor’s appointments, transportation etc.
Group Adult Foster Care Program (GAFC)
A Better Life Home Care provides the elderly and/or disabled clients with daily personal care assistance and light housekeeping tasks. We are able to provide these services via the Group Adult Foster Care (GAFC) program. GAFC is a Medicaid-funded long-term care community option designed to serve clients who meet specific criteria with the goal of providing support services and prevent long-term placement.
We are also able to provide these services throught Personal Care and Home Making ( PCHM) program via Commonwealth Care Alliance program.
•Must be age 22 years of age or older;
•Must meet the financial criteria for MassHealth
•Require a physician’s approval confirming the appropriateness of GAFC services;
•Requires daily assistance with at least one ADL
(e.g., bathing, dressing, eating, continence care, ambulation or transfer);
•Requires clinical approval from the MassHealth and/or CCA Authorization for hours/services
Requires clinical approval for GAFC services from the MassHealth designated clinical screening agent, Coastline Elderly Services, Inc.;
•Client no longer requires assistance with personal care and homemaking tasks
•Client no longer wants to participate in GAFC program;
•Client no longer meets insurance or other eligibility crieteria;
•Client requires long-term care
•Client receiving duplicative services with another PCM Agency ( Stavros or Tempus)
