Here you can find ABC's employee handbook where there is information on topics such as: various processes and policies, PTO, employee conduct, etc. Employee handbooks are updated once a year and ABC requires staff to review annually and sign off on acknowledgement. You can also refer to the employee handbook at your convenience throughout the year.

For all changes to payroll, including address changes, please contact Sophia directly. Below are three common forms you will need for payroll updates. 

• Direct Deposit - You can use this form to update any direct deposit information for payroll. Please send this form directly to Sophia upon completion. Direct Deposit Form

• Ohio IT4 - This form can be used to make any Ohio state tax withholding changes/updates. Please send this form directly to Sophia upon completion. Ohio IT4 Form

• Federal W4 - This form can be used to make any Federal tax withholding changes/updates. Please send this form directly to Sophia upon completion. Federal W4 Form
