Read the Transcript Request Checklist, completing each step will ensure that you have completed the college application process completely.
When paying online, be sure to complete the purchaser's note section, this tells Mrs. Cullum which college to match your payment with in Naviance.
Complete and return a transcript request form to Mrs. Cullum in H115 -
NO Online Payment Request Will Be Processed Without this form.

The Release Form is found in the forms section of the Student and Parent's/Guardian's PowerSchool account, both must be signed and submitted.
NOTE: a laptop, desktop or iPad using a web browser works best when accessing the form section; you can access forms from the APP, it is not as user friendly.

Turn in completed Request form and a copy of the scholarship with proof of payment to Mrs. Cullum
All requests will be available for pick up within 3 business days following submission. Plan accordingly.
All requests will be in a sealed envelope - breaking the seal makes the transcript unofficial.
