The General Internal Medicine (GIM) service at the University of Alberta Hospital (UAH) currently offers 11 services! This is comprised of 7 GIM Wards, an Emergency consultation service, a back-up Emergency daytime consultation service, a GIM Ward consultation service as well as the Virtual Hospital. For teaching rotations at the UAH, we have have learners from students, residents, GIM specialty residents, and elective learners in all of the Clinical Teaching Units (CTUs), Emergency Consult service, and Ward Consult service.
The UAH is considered a quarternary center and is one of the busiest hospitals in the city. UAH serves a large geographic territory including Northern Alberta, and certain regions of Northern BC, Northern Saskatchewan, and the North West Territories. The hospital also provides a variety of specialty services As such, the volume of patients seen by the GIM service is amongst the highest in the city. GIM rotations, therefore, offer trainees an opportunity to participate in a wide variety of General Internal Medicine patient presentations and consultations.
The Department of Medicine currently supports 7 General Internal Medicine (GIM) teams at the University of Alberta Hospital (UAH). There are 3 Clinical Teaching Units (CTUs), which are made up of a team of house staffs including two to three junior residents (R1 or R2) from a variety of programs, and two to three student interns (SI’s) completing their medicine clerkship rotation. If possible, each team also includes a Senior Medical Resident (R3) or Junior attending (R4 or R5). Each CTU team is responsible for the management of up to 25 acute medical inpatients under the supervision and direction of a faculty internist, serving as the attending physician for the service on a rotational basis.
Each patient on the service is assigned to either a junior resident or a student intern, who assumes responsibility for the primary care. Junior residents will be responsible for the primary care of up to 8 patients on the service. Each medical student will be responsible for up to 5 inpatients. The senior medical resident or junior attending on the team coordinates and supervises the management of all the patients on the service, with the guidance and direction of the attending physician.
Virtually all patients on the GIM CTU’s are admitted through the Emergency Department. As far as possible, all CTU admissions are directed to the wards (currently 3 teaching units and 4 non-teaching units) to facilitate a multidisciplinary approach to patient care and medical education.
There are 2 GIM consultation services at the UAH, one that services primarily the emergency department and one that services the other non-medicine inpatient consultations as well as including some ambulatory care component. The GIM Emergency Consultation Service is responsible for providing general medical consultations for patients referred by the Emergency Physicians, including screening all potential admissions to the GIM Wards. The GIM Ward Consultation service screens potential transfers from the ICU to the GIM Wards, as well as providing a wide range of inpatient medical consultations for patients admitted to general surgery, other surgical specialties, psychiatry, and occasionally other inpatient medical services. The rotation therefore offers trainees an opportunity to participate in a wide variety of General Internal Medicine consultations, ranging from the assessment of acute medical emergencies, to the screening and triage of medical admissions, pre-operative assessments, the management of peri-operative complications, and the assessment and management of a wide range of medical problems affecting patients on non-medical services.
