Payroll Google Form
Overview
We utilize our Payroll Google Form to accept all academic payroll appointment requests (students / part-time faculty / researchers / full time faculty). The form is designed to be straightforward and we included ‘help text’ for users. The important notes are:
All academic payroll appointment requests should be submitted via the Payroll Google Form.
Be careful to select the correct “type of appointment” in question 2 as this “type” is used to route the submission to the correct team member(s) for review/processing.
Full time faculty new hires and rehires are first routed to A&S Faculty Advancement for review and approval and then routed to A&S Payroll for processing (see details below).
Researchers and faculty additional compensation are first routed to A&S Academic HR for review and approval and then routed to A&S Payroll for processing
Students and part-time faculty are routed directly to A&S Payroll for review, approval and processing
Please attach the A&S Payroll PDF form separately to the back-up paperwork (such as appointment letters or approvals). This split is for recording keeping purposes at the University.
Submit only one (1) employee at a time. Do NOT bundle multiple employees together into one (1) submission.
The form sends three (3) automatic confirmation emails:
Immediately after submission, a confirmation email is sent to the Departmental submitter
If the submitter is NOT the Department Administrator/Director for the unit, then a confirmation email is also sent to the Department Administrator/Director
Once the submission is approved by A&S Payroll, an ‘approved’ confirmation email is automatically sent to the original submitter
Users must be logged into their NYU Email account to access the form
Full Time Faculty Details
The following process is only for FT Faculty new hires, re-hires or extensions. It utilizes Shared Google Drives/Folders in addition to the Google Form. Instead of uploading one (1) combined PDF file into the Google Form, we ask that you use Google Drive to create a NEW FOLDER for each individual faculty member within your recently created departmental specific "FAS Faculty Google Shared Drive". Add the required PDF files – including the ‘New Hire Form’ -- to the new folder with clear labeling. Please do NOT combine the PDFs and please do NOT use your personal Google Drive to create the new folder. Your recently created departmental specific FAS Faculty Google Shared Drive is listed here.
When completing the Google Form for FT Faculty new hires or re-hires, you will be routed to the required question that asks for the web address (URL) of the new subfolder created by you for the faculty member (see instructions with screenshots).
Submissions for FT Faculty new hires or re-hires first route to Melinda Starmer and Petal Wellington for review before being forwarded to A&S Payroll. Access to the departmental specific "FAS Faculty Google Shared Drive" is limited and are listed here. If you would like to add someone from your department, please email fas.payroll@nyu.edu. Please note that part-time faculty (adjunct) instructors should not be submitted using this method.
