Once filled by the faculty, forward to the student in PDF format (i.e. Save As at PDF document)
The student will complete and send it back by email. After obtaining the student's signature, forward the document to PAS@nyit.edu for filing.

Change of Grade
Submit using the online grade change application in Faculty Center on my.nyit.edu. To access the application, open a grade roster and click on the Request Grade Change button. No chairperson signature is required.
Last date of attendance and the last date of attendance reason are required for grade changes to I, F, W, and UW.
Date work was completed is required for grade changes from I or S to a grade.
You will be required to include the following supporting documentation: attendance record and grade book. The documents may be an Excel spreadsheet, Word document, or Canvas screenshot. They do not accept student’s coursework, master’s thesis, doctor notes, hospital releases, pictures of a COVID-19 testing kit, and the like as supporting documentation. A request to change attendance will be automatically denied if documentation is incorrect.
Change of Attendance
Also complete this online using the Change of Attendance app in Faculty Center. You will be required to include the following supporting documentation: attendance record and grade book. Refer to the Faculty Handbook for a definition of attendance. Faculty must record daily class attendance in a classbook, Excel spreadsheet, or any other method. Per faculty handbook, “the grade book must contain the examination and assignment grades for each student so that the manner in which the final grade was computed can readily be determined.” If a student never attended a class, it is sufficient to submit an attendance sheet and grade book that do not list the student.

IMPORTANT NOTES:
MEMBERSHIPS: The Budget office's directive is that memberships must be paid via the Oracle Requisition system. So, please submit the UNPAID invoice to the Admin, without any reimbursement forms, instead of paying it using your own credit card. However, if for some reason you have already pre-paid, use the above-linked check requisition form, and attach both the invoice and proof of payment.
RECEIPTS: All receipts should be itemized and show the last 4-digits of the credit card used. If that is unavailable, a credit card statement should be submitted.
IF THE DEAN IS PAYING FOR IT: Complete the relevant form and obtain the Dean's signature on it BEFORE the event. For the correct forms, see the "Dean's Forms" section below, or ask the Dean's assistant.
Check requisition form (for non-travel reimbursements only).
Download form (File menu>Download). Complete the blue fields, attach all receipts and submit to the Admin for processing
Expense Report Form (for travel reimbursements, such as conferences and visiting clinical sites)
Download form (File menu>Download). Complete all relevant field, and sign. Attach invoice, ITEMIZED register receipts, organizer's document (e.g. brochures/website/promotional email) regarding the conference (should include location and dates), Google Maps printout to support mileage. Submit to Admin for processing.
