ACCOMMODATION AND TESTING PROCEDURE
If a student comes to you seeking testing accommodation, send them the following two links:

Every semester, the student must meet with the Assistant Director of Accessibility Services in the Office of Accessibility Services (OAS), and present professional documentation to request accommodation.
OAS will send a letter of accommodation to the faculty of each class that the student is taking, detailing the accommodation. The letter must be signed by OAS staff, student and professor, and then returned to OAS.
OAS must be informed of exams at least 5 days in advance, as follows:
For every exam, the student is responsible for downloading the Exam Accommodation Form, completing the top portion and forwarding it to the professor in a timely manner. If the student asks for the form, tell them to contact OAS. Do NOT provide them with the form.
The professor completes the lower part of the form and forwards the completed form to the latest Assistant Director listed for Long Island at the bottom of this page: https://www.nyit.edu/campus_life/accessibility_services (Expect staffing changes)
For each and every exam, it is the student's responsibility to remind the professor of the accommodations they are entitled to.
NOTE FROM CHAIR:
This is a student driven process, and they initiate it with OAS and we sign the form if we agree with it. There is a conversation that can be had with OAS if the accommodations significantly alter the curriculum so be vigilant for that if it should occur.
