Forms & Guidelines

Below are several official departmental and university forms that may be useful to you.

General Student Forms:

You may use the Departmental Student Request Form to make any request which does not require Office of Graduate Studies approval, such as:

Request to waive a requirement
Request a change of advisor
Request an extension (e.g., to submit QP2, MRP, schedule Proposal Defense, etc.)

The Academic Affairs Office of the Joint Office of Graduate Studies ultimately approves certain other requests that impact one's permanent academic record. You can find these forms here for any program changes that are approved by both the Department and the Office of Graduate Studies. Examples follow below. (Your departmental academic advisor must indicate their approval in the "details" field before you submit the request to the Graduate Program Coordinator for DGS review.)

Leave of Absence
Extension of time to Graduate
Advanced Standing
Transfer of Credit
Withdrawal from Program

For Assistantship Recipients:

PhD students on assistantship will use the GA Request for Hourly Work Form to request permission to accept a second, hourly job on campus for a maximum of up to 5 hours per week beyond assistantship duties.
