CARE File Review Guide

Prior to requesting a CARE meeting, please conduct a cumulative file review.

Please note on the CARE referral form:

• Date of File Review

• If the student has changed schools recently/more than once

• Date and results of last hearing check

• Date and results of last vision check

• Any other information such as services received (Title I, ELL, etc.)

• Attendance (attendance report can be obtained through ESIS)

Besides documenting the above information on the CARE referral form, please review all past report cards.

• Note grades and teacher comments. Do the previous years’ report cards reflect similar patterns of strengths and weaknesses that you are seeing in the current year?

Please bring the student’s cum file to the CARE meeting.
