Transition Portfolio

Wachusett students will be asked to complete a Transition Portfolio in order to assist in post-secondary planning.  The portfolio will be used to help students identify strengths, goals, and create action plans. This document will aid in the creation of the student's individual Transition Plan and the IEP. This is a collaborative, fluid document to be updated throughout the student's time at Wachusett. 

If a student requires additional assessments, several can be located on the Transition Assessments page. Below, you will find examples of assessments a student may be asked to complete for transition planning purposes. Every student with an IEP at Wachusett will have a portfolio in Google Drive with activities to complete each year; assessments and activities will be differentiated based on needs.
