Transition planning begins when the student becomes 14. The process involves the student, parents, and IEP Team. Each year the student is enrolled at Wachusett they can take steps to prepare for post-secondary adult life.

Individual Transition Planning Guide - to be completed annually prior to the student's IEP meeting.

Is Your Transition Plan Working? - to be reviewed every six months.

Transition Portfolio - to be completed throughout the student's high school career. The Digital Portfolio is stored in the student's WRSD Google Drive account and can be accessed by liaisons.
