Health Opt-Out Forms

The West Long Branch School District encourages all families to have their child(ren) participate our Board of Education approved Health curriculum, which is guided by the New Jersey Comprehensive Health and Physical Education Student Learning Standards. The district also recognizes that N.J.S.A. 18A:35-4.7 allows parents or guardians to request their child be excluded from specific lessons tied to “health, family life education or sex education” that are in conflict with their conscience or sincerely held moral or religious beliefs.

If you wish to opt-your child out of specific lessons, please complete the appropriate form below. Please note that the forms are grade specific. If you have more than one child in the same grade, please complete an individual form for each child.
