B. Writing Your Questionnaire
Introduce yourself : name, insitution or company, and credentials.
Explain the purpose of the questionnaire.
Reveal what will happen with the data you collect : class project, publication, or research.
Estimate how long the questionnaire will take.
Describe any incentives that may be involved.
Make sure your questionnaire looks professional.
