Incoming Kindergarteners
If your child has a chronic condition, they will need to have an action plan on file with their campus nurse. You can click the appropriate button to the right to download a copy of the action plan.
If you do not see an action plan here that is applicable to your childs condition, please contact the nurse on your campus.
All action plans need to be signed by a guardian and licensed medical professional to be considered complete and active.
