Important Notes:
Keep in mind these could be triggering questions for many students
If a student self-reports lower than a 5 on a sliding scale question, a possible counselor referral is needed.
If a student has a difficult time (beyond appropriate shyness) answering many of these vulnerable questions, a possible counseling referral may be needed
If a student shares any yellow or red flags with you let them know that you WILL touch base with the school counselor, but they can choose if they want to proceed with the counselor
“What are your preferred pronouns?”
“What do your parents/family refer to you as? Are you comfortable with that?”
“Do you have anyone you feel comfortable talking to about this?”
“Do you have outside support? Are you aware of community support/outreach/allies?”
“I see the name you wrote on the packet is different from what I have, is this what you like to go by?”
“Who are three safe people you can go to receive support? Can you tell me at least one school staff you feel comfortable talking to?”
“Is your family supportive of you? Do you want your family to receive support?”
“Do you feel safe at our school, do you feel you have support at our school?”
“Do you know about the school services we provide?”
