What is a sports injury screening?

• Athlete sustains an injury that requires an evaluation by a medical professional. Athlete may be referred by coach, school athletic trainer, or self-refer with parent (if under age of 18).

• Same or next day appointments can be made by contacting the location of choice.

• Athlete receives a Physical Therapy Evaluation and Assessment, along with recommendations for treatment.

• The medical necessity for physical therapy is determined and if required, you may be referred to a physician such as a specialist for further diagnostic work-up and physical therapy prescription

• If additional physical therapy treatment is necessary to assist in a rapid return to play, we require a prescription with a diagnosis from a physician – i.e your pediatrician, urgent care, family or orthopedic doctor.

• If prescribed, aggressive physical therapy is provided to promote healing, restore function and allow a rapid return to play.

• • • • No. However, it is recommended to ensure that you are seen in a timely manner and minimize the wait to be seen.

3. Do I need to see a doctor first?

• • • • No. Physical therapists are licensed to provide physical therapy evaluation and assessment without a physician referral.

4. Do I need to see a doctor for continued treatment

• • • • Yes. Physical therapy treatment requires a diagnosis and prescription. We can provide you with a prescription with the recommended treatment listed on it for your doctor to sign.

5. Will my insurance pay for physical therapy?

• • • • In most cases, yes. Our front desk team will help to verify insurance benefits and if possible please bring your insurance card to assist in the process.

6. Is the parent required to attend the sports injury screening?

• • • • Yes, but if unable due to work or scheduling conflicts the athlete can bring home consent forms and sports injury screening paperwork before their appointment.
