Start Here: QI vs Research checklist
If you think your project is QI, complete This QI Application Form and submit it to the IRB (preferably through IRBnet, or can email to irb@umhsparrow.org). You should also include the checklist and a description of the project (including overall description, what data you wish to gather, what you plan to do with the data, how the project will affect quality at Sparrow). If your project includes accessing patient charts, you will need to include this HIPAA Waiver form.
The Sparrow IRB will review your checklist and study description and let you know if they agree with your plan. If your project is determined to be QI, IRB approval may not be necessary.

Note to submit projects and forms we will use the IRB.net website as described in the above coverletter and check list
Note in order to submit to the IRB the PI and all investigators must have an up to date Human Subjects Training Certificate and CV
