For meetings occurring in the Fall (September) and Spring (March) please complete this form regarding personal and professional goals.
1) Complete this form for patient encounters numbers
2) Review and complete the Semi-Annual Report in NI. Note in the comments section any deficiencies or areas of concern. (this is found in the portfolio, manage reviews section)
3) A Milestone Self Evaluation in New Innovations. This self evaluation lists the sub-competencies the milestones fall under. You will see a short summary statement and be asked to rate yourself as
Novice, advanced beginner, competent, proficient or expert
These levels roughly correspond the the 1-5 numeric levels of each sub-competency and represent the developmental process that occurs through residency. For example, year one residents on average are in the level 1-2 range (novice-advanced beginner), year 2 residents are on average in the 2-3 range ( advanced beginner to competent ) and year 3 residents on average are in the 3-4 range (competent to proficient).
4) ILP- Be prepared to review your individualized learning plans at the meeting. As a reminder your ILP is created and stored as a live document here STFM ILP repository, which is located at: ilp.stfm.org
