10. Disaggregate attendance data, identify possible goal

11. Disaggregate discipline data, identify possible goal

12. Disaggregate report card data (D’s/ F’s and personal, social, academic skills), identify possible goal

13. Disaggregate Early Warning System data, identify possible goal

14. Disaggregate Climate Survey data, identify possible goal

PROBLEM IDENTIFICATION and RESPONSE

15. Present and discuss with stakeholders.

16. Choose a goal (observable and measurable): What are you already doing in your program that you can possibly use to meet the identified problem? What do you need to put in place?

17. Create an Action Plan/Identify effective intervention resources

18. Set start date, send permissions (if needed) give pre-test

19. Implement the intervention (of course if this was a group or series of class lessons, days 20 and 21 would not be sequential)
