Karen is pretty much rid of Epilepsy but in its day, caused her to convulse  at home, at work or in public. People didn't know of her condition so didn't know whether to or how to help or were afraid of helping. So she had Epileptic tattooed on her forearm so people might understand what was going on and assist in some way rather than ignore her convulsions.

The electric overlay is to show what Karen feels when having convulsions and the stark black and white is to add drama to what she goes through.
