SWIM & DIVE REGISTRATION CHECKLIST 

(2025 Season) 

• Register athlete with District 728 (this is separate from booster club contact info. QR code in flyer below

***Make sure your athlete has an updated physical on file with their school!***

• Submit Contact Info for Booster Club (parent’s names/address/email/swimmer’s name(s) for the booster club for emails/updates/contact needs. QR code below, otherwise click link)

• Purchase Suit – $60.00 (required)

• Warm-up -$63.75/jacket (required) $48.75 pants- (optional but encouraged) 

• Personalized Silicone Cap $35 for two- (optional but encouraged)

• 
  

• (Orders are due by July 25 (so we can have suits by pics)

• Apparel (optional)

  (Orders are due on August 6 )

• RHS GSD Booster Fee - $125/athlete (which goes toward coach salary, warmup jacket personalization, team building events,JV/Varsity Section gift bags, senior gifts, coach busses, and more.) 

• Splash Grams (optional -but a great way to cheer on your athlete and raise funds for the team..QR code below)   Due by 8/22

• Lastly, Sports Physicals need to be up to date! To see if you have one on file, you can check with your school OR Log in to Campus/Parent portal, click on More option> Health> Immunizations> scroll to the bottom to see the last sports date that is on file.
