SWIM & DIVE REGISTRATION CHECKLIST (2026 Season)

• Register Your Athlete with District 728 (Separate from Booster Club contact information.) QR code or Link below.

• Verify Your Sports Physical (Required) Make sure your athlete has a current physical on file.

  • Check with your school, or log in to Campus Parent Portal → More → Health → Immunizations and scroll to the bottom to view the sports physical date.

  • Sports physicals are valid for 3 years.

• Complete the Booster Club Contact Form (Required)

  • •  Submit your family's contact information to receive important team emails, updates, and Booster Club communications. 

      • QR code or Link below. 

• Place Your Team Order Store is Closed.

  • • ✔️ Team Suit (Required)

    • ✔️ 2 Personalized Silicone Caps (Optional, but Encouraged)

• Apparel (optional) Store is Closed.

• RHS GSD Booster Fee - $125/athlete (which goes toward coach salary, warmup jacket personalization, team building events, JV/Varsity Section gift bags, senior gifts, coach busses, and more.) QR code or Link below.

• Lastly, Splash Grams – $15 Each (Optional)
