1. For the purposes of reporting for the grant, share the link below with your principal to help us gather information from their point of view about the Reaching Out Program: Principal Survey
2. Also, for the purposes of reporting for the grant, share this link with one teacher you have collaborated with in any way since the beginning of the Reaching Out Program: Teacher Survey
3. Browse through your Digital Journal to make sure you have completed all requirements from Units 2 and 3.
4. Use your last page in your journal to write a two or three paragraph summary of your hack/goal accomplishments. (Optional: Create your summary in this Flipgrid video and note it in your journal.) For example, you may want to include...did it go as expected, did you need to adapt your original plan or timeline, what would you do differently, was there pushback to overcome, did you form any new relationships, what student learning occurred, what was the best thing that happened, what might you try next, etc.
5. It's OK if your hack/goal is not yet totally complete by December 1. Reflect on what has been accomplished and what needs to happen next.
