1. Doctor’s referral letter.

2. Copy of main member’s ID.

3. Copy of medical aid card.

4. Previous relevant X-rays / sonar / scans.

5. Completed information form.

6. Completed consent form.

• Please arrive 15-20 minutes before your appointment to complete patient information and consent forms.
