Forms for Thesis/Dissertation

As a graduate student prepares one's research manuscript, there are certain protocols and documentary requirements that need to be accomplished in every step of this academic journey. You can download and edit the Word Format of the forms needed by clicking on this link, or you can have PDF copies of these forms here.

Here are the general steps and forms you need to prepare and secure:

  1. Before your Defense (proposal or final)

  • Send manuscripts (black ring-bound with clear plastic cover clearly showing the title page) to your confirmed panelists one week (or 5 working days) before your scheduled defense date.

  • Send the invitation letter [Form 10] (duly signed by your adviser) together with your manuscripts and secure the reply slip (found at the bottom half of the letter). This is your proof that the panelists agreed to sit in your presentation.

  1. During the Defense

  • Prepare the following forms:

    • Evaluation Sheet [Form 11] to be filled out by your adviser during the defense deliberation. This should be turned over to the student and be used as a guide in fulfilling the revisions required.

    • Adviser's Rating Form [Form 12 or Form 22] should be filled out by the adviser and should be turned over to the GS Secretary for recording.

    • Prepare several copies of the Panelist's Rating Sheet [Form 13 or Form 23] - 3 copies of the form for panelists in a master's thesis and 5 copies of the form for panelists in a doctorate dissertation. These forms should neither be turned over to the student nor the adviser. These forms should directly be sent to the GS Secretary to be collected and compiled for recording.

  1. After the Proposal Defense (before collecting data)

  • Have the panelists and adviser sign the Clearance Form for Revised Proposal [Form 14]. The Student should keep this form secure as this serves as proof that the panelists and adviser have already given their permission and allow the researcher for data gathering procedures to commence.

  1. For the Final Defense

  • Repeat Steps 1 and 2

*Use and prepare Form 21 instead of Form 11, Form 22 instead of Form 12, Form 23 instead of Form 13

  1. After the Final Defense

  • Use and prepare the Clearance Form for Revised Final Draft [Form 24]. Again, the student keeps this form as proof.

  • Comply with the requirements of Form 24. For the Certification [Form 25], have the respective administrative official affix their signature beside the appropriate item/s (i.e. have the Research and Publication Office check the format, submission of CD copies, etc.). The student keeps this as proof as far as the Thesis/Dissertation course is concerned.

Internship

Internship is designed to expose graduate students to a variety of research experiences in the various field of work of the discipline. It is, therefore, a transition point from the “academic” to the “real world” in terms of applying theories into practice or action. The Master of Arts degree in Filipinology at the Graduate School of the Pamantasan ng Lungsod ng Marikina requires an internship placement in any of the four areas: advocacy, policy-making, educational, and community/cultural settings.
