STEPS
1.Download the form here. Or contact our staff above and we will mail a printed form to you.
2.Once filled, email the form back to us, or mail to PACCAL at 380 Monmouth St. Jersey City, NJ 07302.
3.We will notify you via email or phone once the vouchers are ready. You may pick it up at our center at 380 Monmouth St. Jersey City, NJ 07302
4.If you lose your voucher, notify us so we can help get you a replacement.

ELIGIBILITY
- Jersey City senior residents
- Aged 60+ who meet the SFMNP income requirements.
- Applications are available in late May.

Distribution of vouchers from July 1 thru September 30.
Vouchers are valid through November 30.
