The parent/caregiver form has different questions than the educator form as the educator form is relevant to the pediatric client's sensory processing in their educational setting.

The Sensory Profile should take about 5 minutes to complete. It is individually assessed. There are forms for care-giver/parents and for teachers to assess their interpretation of the pediatric client's sensory processing and interaction with their environment. The sensory profile can be administered
Occupational therapists are the best professional background to be administering this assessment to to educational training with sensory processing effects on daily living, but psychologies, speech language pathologists, physicians, and science researchers can administer this test.

The parent/caregiver completing the assessment marks answers to questions based on frequency of observing behaviors on a scale of 5 being "Almost Always - 90% or more," 4 being "Frequently - 75%," 3 being "half the time - 50%," 2 being "Occasionally - 25%," 1 being "Almost Never 10% or less," and 0 being "Does Not Apply."
Sections are broken up into "Auditory, Visual, Touch, Movement, Body Position, Oral Sensory Processing, Conduct, Social Emotional, and Attentional."
Raw Scores are added up by Quadrant labeled with "Seeking, Avoiding, Sensitivity, Registration" and by Sections for "Auditory, Visual, Touch, Movement, Body Position, Oral Sensory Processing, Conduct, Social Emotional, and Attentional."
A normative data curve is provided to assess how the client compares to others on a bell curve.
Where the scores land on the bell curve determines the client's sensory processing on a scale that is "Much Less Than Others, Less Than Others, Just Like the Majority of Others, More Than Others, and Much More Than Others."

Based on the Normative data from the Sensory Profile's bell curve a client scoring "Just Like the Majority of Others" would bein in the percentile range between "20-47 for Seeking/Seeker, 21-46 for Avoiding/Avoider, 18-42 for Sensitivity/Sensor, 19-43 for Registration/Bystander, 10-24 for Auditory, 9-17 for Visual, 8-21 for Touch, 7-18 for Movement, 5-15 for Body Position, 8-24 for Oral, 9-22 for Conduct, 13-21 for Social Emotional, and 9-24 for Attentional." There is adequate interater reliability for 60 parents completing the Sensory Profile Caregiver Questionnaire for children ages 5 to 10 (ICC = 0.48) (Cite Shirley Ryan).

The Child Sensory Profile demonstrates a good way of understanding a child's sensory needs to determine a plan of care and sensory processing strategies and tools to support functional participation in their environment.
As this assessment is based on the parent/caregiver, or educator's observation of the child, there can be bias when answering questions and differences of how the client interacts with their environments from school and home. The OT administering the report must provide good eduation on how to complete the form prior to administering to prevent bias.
