Screening tool for measuring functional cognition that may impact performance in attention, sensorimotor skills, visual-motor, executive functioning, and problem solving in order to support making OT interventions based on their performance level in their plan of care.

Assesses attention, problem-solving, new learning, and ability to follow directions based on the Allen Cognitive Levels (0.0–6.0).

ACLS-5 Manual
Involves 3 stitching tasks going from easiest to most complex (running, whipstitch, and single cordovan) using leather, lace, and tools. The administrator has to follow specific prompts and cues when giving directions and demonstrations for this assessment. The administrator must demonstrate the stitch while giving the verbal directions initially. The manual provides great detail about what to say, when to note the behaviors, and responses to errors.
Possible cues include, "Do your stitches look like mine?," "Please try to fix them, "Just do the best you can."
Scored based on ability to replicate stitches accurately and problem solving skills when making errors. The scores can range from 3.0 to 5.8 which corresponds with Allen's Cognitive Levels.
Good interrater reliability across ACLS and LACLS tests from the years 1978 to 2007.
Less reliable test-retest consistency due to fluctuating cognitive performance based on the client being tested.
Excellent construct validity with testing functional cognition associated with global cognition from the MMSE.
Highly reliable test that follows a detailed standardized testing manual to follow. Correlates with the Allen's Cognitive Disabilites FOR.
Looks overwhelming and there is a need to follow the exact procedure during testing. Need to be trained prior on how to complete the stitch. This test may not be reliable for someone who is an expert in stitching tast.
