All coaches have cleared a 3rd party background check by Sterling Volunteers.

Head coaches are First Aid and CPR certified by the American Red Cross.

All NDR coaches are YCADA Certified.

Fall 2022 Registration

• There are FOUR forms that must be submitted for registration.

1. 2022 Athlete Information Sheet

2. 2022 Pop Warner Participant Contract and Parental Consent Form

3. 2022 Pop Warner Physical Fitness & Medical History Form

   • MUST be completed by doctor AND parent / guardian.

   • This form may be dropped off to be completed by medical professional and picked up at a later date; athlete physical may or may not be necessary.

4. Mr. Todd's Gymnastics Waiver

• Print forms. Complete forms. Scan or take a picture to email.

• Completed forms must be emailed (scans or photos) to jo.ndraiders@gmail.com AND physically submitted in person.
