The COVID-19 vaccine mandate will be ending on Tuesday, May 23, 2023 for 22 CUNY Colleges and for CUNY Central Office employees. Please note, approved exemptions to the vaccine mandate will also end on the dates noted above.

Complete form Vaccine Status and Vaccine Information
IMPORTANT
Those CUNY employees who are not fully vaccinated or who choose not to disclose their vaccination status will be required to submit to COVID-19 testing.

Only complete NYS Excelsior Pass information if you have an Excelsior Pass and a screen shot of your pass. If you do NOT, PLEASE LEAVE BLANK.
