Forms & Requests

This is the hub for all lab-related forms. Whether you want to suggest an update to the website, request a change to your info, or get connected to the lab for the first time—this is where to do it.

How to use these forms (quick guide)

• Be specific. Paste exact text to add/change or include direct links/citations.

• Uploading images/files? Use the upload field in the form. If your file lives in Drive, share-link it.

• Multiple requests? Submit once per page/topic so we can track and close the loop cleanly.

What to expect next

• You’ll get a brief confirmation on submission.

• A lab site maintainer will review and follow up if we need anything else.

• Accepted updates get queued and published during the next edit window. 

• If something is time-sensitive—conference deadline, correction, etc.—mention it in the form.

Accessibility & privacy notes

• Forms are hosted on Google Forms. If you need an alternate way to submit (email, Discord DM), let us know and we’ll accommodate.

• Submissions are visible to the site maintainers only. We use your contact info solely to follow up on your request.
